Booked CDK Walk-In Services at Sutherland
We are now providing walk-in (WI) services in the form of book appointments with limited providers and availability at CDK Sutherland Drive.
This option allows patients without a family doctor to book appointments. This must be for a medical reason. Any bookings for MEET AND GREET/ NEW DOCTOR will be removed. We also do offer Tb testing, same day or acute medical concerns (must not be emergencies - limit 1 issue per visit), MTO Physicals, WSIB forms, Pap smears, Baby vaccinations, Prenatal Care, Preventive Health Visits, Chronic Disease Management, and Third-party physicals via this method. Forms and general assessments that are required by a third-party are subject to fees.

Please fill out all of your information, including your name, contact phone number, email address, and health card information, in order to confirm the appointment booking. Any bookings without complete contact information will be removed.
YOU MUST HAVE A VALID HEALTH CARD on the date of the appointment, or else you will pay a fee for the medical visit and services provided. You will also be charged if you miss an appointment. See below Missed Appointment Policy.
Appointments solely for Meet & Greet or for a new doctor will be removed without notice.
Please note that if you miss an appointment, or cancel with less than 48 hours' notice, then there will be a charge for a missed appointment, and you will lose the privilege to book another appointment at CDK or see a CDK doctor at any clinic site until your fee is paid. Furthermore, you will not be able to roster with a family doctor (be accepted as a new patient) until any outstanding fees are paid.
