This option is NOT intended for Tb testing; MTO Physicals; WSIB forms: Queen's or St. Lawrence students who can access primary care through student wellness clinics or people who are rostered to a primary care practice.

Booked CDK Walk-In Services at Sutherland
We are now providing walk-in (WI) services in the form of book appointments with limited providers and availability at CDK Sutherland Drive.
This option allows patients without a family doctor to book appointments for Tb testing, same day or acute WI medical concerns (must not be emergencies - limit 1 issue per visit), MTO Physicals, WSIB forms, Pap smears, Baby vaccinations, Prenatal Care, Preventive Health Visits, Chronic Disease Management, and Third-party physicals.
YOU MUST HAVE A VALID HEALTH CARD on the date of the appointment, or else you will pay a fee for the medical visit and services provided. You will also be charged if you miss an appointment. See below Missed Appointment Policy.
If you do not have a valid health card, please book using the KCHC booking link in OPTION 1, ABOVE.
Please note that if you miss an appointment, or cancel with less than 48 hours' notice, then there will be a charge for a missed appointment, and you will lose the privilege to book another appointment at CDK until your fee is paid.
